After almost 45 years in private practice, I will be retiring from Gastroenterology Consultants of San Antonio, effective December 31, 2020. It has been a great pleasure and honor serving as your physician over the years.
You may continue your medical care with any of my partners at our Hamilton Wolfe location. You may also consider transferring your care to one of my partners who offices in one of our other locations more convenient for you.
Your patient records will remain with GCSA, and rest assured that your care transition will be handled with the greatest concern for your health and privacy. I’d suggest you visit our website at gastroconsa.com, where our physicians profiles are listed.
However, if you choose, your medical record can be transferred to another physician’s office upon receipt of a signed release authorization. A transfer is not necessary if you wish to continue your care with another physician at GCSA. If you have any questions, please feel free to call the office at 210-614-1234.
I once again want to thank you for your trust, support, and friendship. I will miss that immeasurably.
