What Is the Liver?

The liver is a large organ located in the upper right side of your abdomen, just under your ribs. It performs jobs every day to keep your body functioning, including:

  • Producing bile to help digest fats
  • Processing nutrients from food
  • Regulating blood sugar and cholesterol
  • Making proteins that help with blood clotting
  • Breaking down medications and toxins

When fat builds up in the liver, it can cause swelling and damage to liver cells. Medically, this is known as metabolic dysfunction-associated steatotic liver disease (MASLD).

Eventually, this can turn into a more serious condition called metabolic dysfunction–associated steatohepatitis (MASH). It’s a more serious form of fatty liver disease that can lead to liver scarring as it progresses.

Why Are Fatty Liver Symptoms Easy to Miss?

The liver is remarkably resilient. It can continue to do its job even when fat or early damage is present. Because of this, MASLD often has no clear symptoms at first. You may think you’re just stressed or not sleeping well.

When liver swelling gets worse, you may start to notice signs that your liver is not working properly, such as:

  • Yellowing of the skin or eyes, known medically as jaundice
  • Persistent itching without a clear cause
  • Spider-like blood vessels on the skin

These symptoms suggest the liver may be struggling to process bile or maintain normal blood flow.

If MASH leads to cirrhosis, or long-term liver scarring, the symptoms are often more severe and easier to see, such as:

  • Fluid buildup in the abdomen or legs
  • Easy bruising or bleeding
  • Loss of muscle mass
  • Confusion or memory changes
  • Difficulty concentrating

Why Don’t Symptoms Tell the Whole Story About Fatty Liver?

The severity of symptoms does not always match the severity of liver damage.

Some people with serious liver scarring may feel fine, while others with mild disease may feel sick. That’s why symptoms alone are not enough to diagnose or rule out fatty liver disease. To understand what is really happening, your doctor will perform:

  • Blood tests
  • Imaging
  • Clinical exams with follow-up

It is important to protect your liver and see your doctor if you are experiencing:

  • Persistent fatigue or weakness
  • Ongoing abdominal discomfort
  • Unexplained weight loss
  • Yellowing of the skin or eyes
  • Swelling, bruising, or changes in thinking

Fatty liver symptoms can be easy to miss at first. You might feel tired. You might notice stomach discomfort. Or you may not feel anything at all.
